چکیده

Objectives: To compare the outcomes of patellar replacement and neurectomy in Patients undergoing Total Knee Replacement . Materials & Methods: The design this study was a cross sectional conducted DHQ Teaching Hospital Gujranwala. 30 were enrolled they divided into two groups. Group A constitutes 15 underwent Post tibial neurectomy. And B consisting went through Tibial Resection. All patients have been assessed Tinel test on painful area self-administered Patient-Reported Outcome Measurements (PROMs): Patellar also known as patellofemoral arthroplasty, is surgical procedure that performed conjunction with total knee (TKR) to relieve pain joint caused by arthritis or other conditions affecting patella. given anesthesia numb area. An incision made front expose patella surrounding structures. Removal patella: carefully removed from joint. underside reshaped fit prosthetic component implanted its place. attached surface using bone cement. repositioned femur sutures. closed sutures staples. goal reduce improve function group participants involves cutting removing nerve endings supply sensation Results: participated study. In =15 who Neurectomy experienced Tibia 5 months follow up done. showed excellent Results. NRS score decreased 0 B. While still moderate pain, Remaining 13 although did not but felt numbness limbs. Practical Implication: patella, interrupting transmission signals brain. Although it less time consuming can cause altered skin over which may affect patient's quality life Does address underlying instability misalignment, functional limitations Can be associated risk complications such neuroma, infection, stiffness. Conclusion: Resection TKA has better than Neurectomy. denervation electrocautery effective reducing opioid consumption after replacement. Postoperative range motion significantly group, means improves clinical outcome Keywords: Replacement, Neurectomy, Resection, Pain, Prosthesis

Anterior knee pain in rheumatoid patients after total knee replacement. Possible selection criteria for patellar resurfacing.

Postoperative anterior knee pain was evaluated in a consecutive series of 138 knees in 108 patients with rheumatoid arthritis treated by total knee replacement with Mark I Insall-Burstein prostheses. No knee had primary patellar resurfacing, and in the 119 knees followed up for a mean of 63.9 months, none had secondary resurfacing. Anterior knee pain was absent in 87 knees (73%), mild in 16 (13...

Electrocautery of the patellar rim in primary total knee replacement: beneficial or unnecessary?

The management of the patella during total knee replacement is controversial. In some studies the absence of patellar resurfacing results in residual anterior knee pain in over 10% of patients. One form of treatment which may be used in an endeavour to reduce this is circumferential patellar rim electrocautery. This is believed to partially denervate the patella. However, there is no evidence o...
